If a particular server is known for sending out too much spam, it gets a lower reputation. If it only sends out “good email” then it has a high reputation score. Even if the email comes from a “good server” it is also scanned for content, attachments, formatting, and thousands of heuristical elements that contribute to the overall spam score for that particular email.
